Zylus Pre-Qualification.

This questionnaire is intended as a basis for establishing the qualifications of Vendors for undertaking Material Supply under the joint partnership scheme of Zylus Group International.

If a Vendor has not submitted to the Qualifying Committee this form setting forth his qualifications to the satisfaction of the committee, the Vendor shall be ineligible to receive Request For Quotation (RFQ) documents for bidding or for Purchase Order (PO) award for such supply as may be handled through the Committee.

Vendor Appraisal shall be conducted on all qualified vendors annually and any Vendor found performing below expectations shall be disqualified and deregistered from the system. If such Vendor seeks to re-apply, the vendor will have to submit to the entire Pre-Qualification exercise afresh after submitting a performance bond as determined by the management of ZGI. Appraisal of Vendors are based among other factors; Lead Time, Quality of Supplies, Conformity to Required Specifications of Materials/Items as agreed on PO.
